Person centred planning

Person-Centred Planning is a process for helping the people we support to plan their lives now and in the future. Carr-Gomm is introducing this approach because we believe it is a more empowering way to enable individuals to achieve their potential than our current models of support.
Unlike traditional support plans, PCP puts the individual in charge of planning and treats them as a whole person with talents and dreams as well as needs and it offers a very empowering and creative approach to planning for everyday life and the future.
PCP is a well-established way of working with people with learning disabilities but Carr-Gomm is piloting the approach with different client groups; including homeless people and people with mental health problems. We have set up a network of staff to champion Person-Centred Planning within Carr-Gomm
Fifteen staff facilitators have been trained to use the tools of PCP, which they are now testing with service users. Their work and findings will be communicated across Carr-Gomm through further training and materials. Each facilitator will share their knowledge and help champion the PCP approach within their region over the next year.
With funding from the Department of Health we are developing this pilot and will share our knowledge with other organisations in 2005.
